How to Make a Request About Your Personal Data

As a global company, CSL handles all personal data in a manner that respects the rights and privacy of individuals, in compliance with various legal and business requirements. Individuals residing in certain jurisdictions have specific legal rights related to the personal data that CSL processes (i.e., those residing in a European Union country have rights under the General Data Protection Regulation). CSL is committed to being transparent about the way we process your personal data and allows all individuals to make a Data Subject Request (“DSR”) related to the personal data stored or processed by any CSL entity or affiliate. CSL may not be able to grant all DSRs as requested, but we will evaluate and respond to each DSR in accordance with our legal and business requirements.

What is personal data?

CSL considers personal data to be any information relating to an identified or identifiable natural person, directly or indirectly, in particular by reference to an identifier such as a name, an identification number, location data, an online identifier or to one or more factors specific to the physical, physiological, genetic, mental, economic, cultural or social identity of that natural person.

How can I make a request?

To make a request regarding your own personal data or, if authorised, the personal data of another individual, CSL asks you to submit a DSR form. This form will provide us with details of the personal data collected and processed at CSL so we can best respond to your request. You will be requested to provide verification of your identify (e.g., a government-issued identification, such as your passport or drivers license) and, if applicable, proof of your authority to make a request on behalf of another individual (e.g. birth certificate for parent’s request about a child’s data, power of attorney for an incapacitated person, written and signed authorization with accompanying proof of identification of the individual). Failing to provide such documentation may delay CSL’s ability to respond to your request.

There are two ways to submit your request to CSL:

  1. Secure Online Form: CSL offers a secure online form to collect and transmit your information to our Privacy Office. Click the button below to complete our online DSR form. The form will allow you to securely attach verification of your identity and, if required, proof of your authorization to make a request on behalf of another individual.

  2. Printable Form: If you are unable to complete our online form, you may download a printable form and post it directly to the CSL Privacy Office.

    1. Print and complete a copy of the appropriate Data Subject Request Form. There are separate forms for making a request about your own information and for making a request on behalf of someone else as an authorised representative.

    2. Enclose verification of your identity and, if required, proof of your authorisation to make a request on behalf of another individual.

    3. Post your completed form and supporting documentation to the CSL Privacy Office. We suggest that you send your information using a tracked postage service that provides delivery confirmation.
